Print Header and Footer, new fields.

I see on the header the path/file name on the left and the current date time on the right. I see on the footer the page number. I see how to adjust these and add some, but not these two new fields. I would like to keep current date/time as that is the print time; but would also like the last modification to the file date/time. I would also like to print Page x of x, not just -x-. like Page 1 of 10, Page 2 of 10… Page 10 of 10…
